AI-Derived CD8⁺ T-Cell Signatures from Tumor Slides Predict Immunotherapy Benefit in Lung Cancer

Immunotherapy has transformed the treatment of advanced non-small cell lung cancer (NSCLC), but not every patient benefits, and PD-L1 testing remains an imperfect guide for deciding who should receive immunotherapy rather than chemotherapy.

This study introduces VIGOR-CD8, an AI pipeline that reads routine baseline H&E slides and predicts the spatial pattern of CD8+ cytotoxic T cells in a tumor.

Biomarker-positive patients lived longer on nivolumab + ipilimumab than on chemotherapy while biomarker-negative patients showed no difference between treatments.

An interpretable immune signature derived from standard H&E slides could help identify patients most likely to benefit from first-line dual immunotherapy; prospective validation is warranted.
